All Out of Network Insurance Patients (PPO and POS)
You can call us at 212-717-8800 to inquire about eligibility.
You can fax us at 212-717-1788 with your insurance information (please fax the front and back of your insurance card) along with your date of birth and contact information. We will check for you if you are covered and have out of network benefits.
Patients must bring their insurance card, major credit card and Photo ID. (to keep on file)
Out of network patients are responsible for their out-of-network deductibles.
Whenever feasible, we accept assignment for the office visit, any in-office procedure and lab work, provided, that the patient has out-of-network benefits. For information regarding your deductible, please contact us at 212-717-8800. We will accomodate you as much as possible.

The initial consultation costs $630 (lasts 60-120 minutes) and follow up visit costs $360 (lasts 30-60 minutes)
